"Getting to know you..." - Neighbourhood Care Networks & Third Sector Caerphilly Netwo

The Aneurin Bevan University Health Board and local partners' ambition is to create a new system of primary, community care and wellbeing across Gwent, in partnership with local government and the third sector. Collaboration with partners will build on the existing innovation across Gwent, and use the Neighbourhood Care Network footprint, as the basis from which local services will be planned and delivered, around a model of community wellbeing.
The purpose of this networking event will be to strengthen relationships between the third sector and the three Neighbourhood Care Networks (GP Clusters North, South and East) by sharing intelligence about what third sector services and community support exists and how we can work collaboratively to deliver key priorities to support the population of Caerphilly Borough.

Social Prescribing and wellbeing support
Provision of Care Closer to Home
Working together to mitigate Winter Pressures
There will be opportunities for exhibition stands to showcase third sector Health Social Care and Wellbeing support.
