Gwent CVC Small Grant Fund - Winter Resilience 2023-24



Expression of Interest

This funding opportunity is open to organisations operating within Monmouthshire and Newport, due to the remaining unallocated funding from the RIF Small Grant Scheme, for these localities. Expressions of interest are now invited, offering third sector organisations to propose initiatives that can offer support within either of the two priority categories outlined below.


Priority 1: Preventative Community Capacity (wrap around support to prevent escalation of

need/specialist intervention)

Priority Intent : Strengthen the community preventative offer, reflective of the pareto principle whereby 80% of the community activity should be a generic prevention model, providing 20% as more specialist services, such as Frailty Rapid Medical, etc.

Examples of third sector activities (but not limited to)

· teaching the community about health and well-being

· offering mental health first aid training

· providing support for overall well-being

· helping with housing and preventing homelessness

· encouraging physical activity to support health conditions

· ensuring homes are safe to prevent falls

· supporting families

· keeping older people in the community engaged and connected


Priority 2: Alternative Capacity to support discharge (a range of solutions to support

individuals waiting for community care to leave hospital/turnaround at the front door)

Priority Intent: Develop a range of solutions that can support individuals to leave hospital when waiting a package of care for a short term period.

Examples of third sector activities (but not limited to)

· coordinating care in the community

· arranging transportation for medical appointments

· offering respite care

· helping with rehabilitation at home or in the community

· educating the community about health

· delivering meals/essentials to support individuals to remain at home

· provide social support and befriending to mitigate crisis interventions

· assisting in crises and responding to urgent situations

· offering legal help
