Successful place-based working needs organisations involved in supporting care and wellbeing in the community to work together to provide the right care in the right way. More community-based working will lead to better outcomes for those accessing care and support

To help develop a Newport plan for delivering the Place Based Care model in Newport, partners need to understand the workforce paid and unpaid available locally to enable people to access support to improve their health and wellbeing. Your role is valuable and vital to care, support and wellbeing in our communities across Newport. The Survey can be found here Place Based Care Resource Mapping

NOTE: The date for completion has been extend through to the end of August to allow for the summer leave period. If you can complete the survey by 31st August your contribution will be a vital addition to understanding the community paid and unpaid support available.
